This study will evaluate whether three patient-reported questionnaires on mental health and phenotype categories (patient subgroups) are suitable for use in the Complex Chronic Disease Program (CCDP) clinical population. This will involve recruiting CCDP patients to complete questionnaires at two time points and then analyzing and comparing the proposed mental health and phenotyping questionnaire scores to previously used generic measures.
The measures that will be assessed are the NEURO-QoL Anxiety Short Form, NEURO-QoL Depression Short Form, and the Patient Phenotyping Questionnaire Short Form. This study would help fill the gaps in available instruments that exist for mental health assessment and phenotyping categories which are also free for researchers to use.
